You can download the Credit Card Authorization Form in the Word Format format. Complete the attached Credit Card Payment Authorization Form, signed by Credit Card Holder and Fax the same along with a photocopy of both sides of the Credit Card & photocopy of Driving License/Passport to Fax No. 310-268-0022 and also inform the details by email to customercare@jimbang.com.
